22-year-old single and no children presented with Abdo distension and pain.
22-yr old. Single and no children. Presented with abdo distension and pain. Pre-op CT did not show metastatic disease. At laparotomy, she had >5L ascites, bilateral large cystic ovarian masses completely replacing both ovaries, extensive dx involving the whole of the right hemi-diaphragm, omentum, abdominal peritoneum, the whole of the pelvic peritoneum and pouch of Douglas/rectum. Also had disease involving the caecum and terminal ileum. We were able to completely resect all the disease to R=0 and preserved her uterus, right Fallopian tube and some amount of normal ovarian tissue. Total operating time was about 8 hours.
